The question marks are usually an indication the the problem lies in the email client receiving the message (which cannot parse UTF-8 encoded subjects), rather than the Send Email plugin.
To test this try sending to other email clients (including a web-based client).
You can also have a look (using trace, or by sending it to the display) at the Delivery Status structure returned via the three exits of Send Email - The 'Delivery Message' element there will contain the raw, mime-structured message sent to the mail server, in which you can verify that the subject is encoded in UTF-8.
