The model I suggested is not intended to return the element name of a given atomic element, but rather to return a list of the fields (name + value) in a given composite data structure.
In your case, you should pass it the database record (rather than each of the data elements in it), and my suggested model whould return a repetition of Field structures, including one where Name=SC, another where Name=DD, etc.
It is up to you to decide what you do with the returned Field data. Hazarding a guess based on the screenshot you provided, you'd probably want to convert each Field structure returned, into a Row ...
